Duel arena:
when you die sometimes you actually die, loosing all items. Another wierd thing about this bug is, when you die your chat log is spammed with messages saying, you dropped brains, crybows, black axes, etc. (items you don't even have) and it treats it as a normal death, keeping only 3 items.
